The ways to die and endings to Mistrick by Hatoge, a horror-ish RPG Maker game where you find yourself in a sort of purgatory with your former cellmate and serial killer and you're trying to escape. It's rather short (probably 40-60 minutes to fully finish). The third ending requires getting all 13 unique deaths (the bear and tree have two different ways to trigger their death).
