Lisa Leypoldt, MD, University Medical Center Hamburg, Hamburg, Germany, discusses the management of triple-class refractory patients with multiple myeloma (MM) in the absence of immunotherapies. Classical therapies such as immunomodulatory drugs (IMiDs), proteasome inhibitors, and monoclonal antibodies are the first options to treat myeloma. If patients are refractory to these, agents with different mechanisms of action should be considered, such as selinexor or belantamab mafodotin.
