Insights into the role of novel BTKis in the treatment landscape of R/R CLL

Danielle Brander, MD, Duke University School of Medicine, Durham, NC, discusses the integration of novel BTK inhibitors (BTKis), namely second-generation covalent BTKis such as zanubrutinib and acalabrutinib and the third-generation non-covalent BTKi pirtobrutinib, into the treatment landscape of relapsed/refractory (R/R) chronic lymphocytic leukemia (CLL). For now, covalent BTKis are favored in patients who have not received prior BTKi therapy or who relapse after treatment with ibrutinib. Non-covalent BTKis are predominantly used after covalent BTKis or venetoclax-based therapy. Dr Brander highlights that further research is needed to better understand how to best sequence these agents in clinical practice.
