When to start oral anticoagulants after ischemic stroke

Senta Frol, MD, PhD, University Medical Centre Ljubljana, Slovenia, talks on the standard practice guidelines for initiation of anticoagulation after acute ischemic stroke. The “1-3-6-12-day rule” is a well-known consensus, advising increased delay of anticoagulation initiation with increasing stroke severity. For a transient ischemic attack (TIA), anticoagulation can be started after 1 day, day 3 following a mild stroke, day 6 following a moderate stroke, and on day 12 following severe stroke. Dr Frol also highlights the importance of conducting CT before initiation in moderate-severe cases, to rule out hemorrhagic transformation. This interview took place at European Stroke Organisation Conference 2022 in Lyon, France.
