It is a non-focal Orbicularis oculi reflex. aka supraorbital, trigeminofacial, McCarthy’s, or nasopalpebral reflex, Myerson’s sign . Procedure: First of all, stand on the side of the patient. Tapping should be done with the reflex hammer/finger from above to avoid eliciting the blink reflex to threat. Tapping over outer aspect of supraorbital ridge, over glabella, or around orbital margin; It can sometimes be elicited by tapping the forehead as far back as the hairline. Response: Tapping causes eyes to blink. Normally,the adult patient habituated to the stimulus and ceases blinking after a few taps. If blinking persists,it is abnormal. Reflex arc: The afferent sensory signals are transmitted by the trigeminal nerve to the brain stem. The efferent signal goes to the orbicularis oculi muscle via the facial nerve, causing the muscle to reflexively contract ,yielding blinking. Significance: It is one of the frontal release signs . It is normal in infants, disappear with brain maturation allowing inhibition. This primitive reflex reappears in disorders that affect frontal lobes.
