A common cause of TMD: Suprahyoid muscle clenching

After working with many chronic jaw and neck patients, I have noticed that they have a tendency of clenching their suprahyoid muscles in order to stabilize the neck. The hyoidal muscles, because of their attachments between the anterior mandible, hyoid and sternum, may aid inn pulling the chin down, similar to the deep neck flexors. Dysfunction of the latter may cause the patient to compensate for such instability of the neck by clenching the throat. This may also be a stress response. Either way, when clenching, and although it will somewhat improve cervical stability, it will cause compression of the TMJ as well as disengagement of the cervical muscles due to loss of stimulus (especially anterior cervical muscle complex, such as the SCM or scalenes). The further the head comes forward, the harder the hyoidal muscles will pull the jaw backwards (posteriorly) and thus the greater the shearing forces within the TMJ will become.
