64 years old male , a known case of hypertension , diabetes , coronary artery disease, chronic obstructive airway disease, b/l bronchiectasis , inflammatory bronchiolitis after being consulted at multiple hospitals in region with no response but deterioration of condition, presented to AIMSR in gasping state with unrecordable BP, loss of consciousness and SpO2 56 percent on room air. Patient was intubated immediately, was put on ventilator with 100 percent Fi02 , started in triple isotopic support to maintain BP, empirical antibiotics and all other supportive treatment on lines of severe sepsis with septic shock with multi organ failures ( AKI, cougulopathy , liver dysfunction, encephalopathy ) in background of above mentioned co-morbidties. Therapeutic plus diagnostic bronchoscopy was done in view of patient not maintaining on even full ventilators support and copious secretions with collapse. Patient was given best possible critical care support. He improved significantly with the management, weaned subsequently and was discharged in stable condition.
