Fractional CO2 Laser is a Good Wrinkle Treatment, but can Thin the Skin if Overused

A woman had upper and lower eyelid surgery 10 years ago. Presently, she says she has a lot of extra skin under her eyes. Her doctor suggested CO2 laser but she is unsure if this is the right thing for her. Dr. Amiya Prasad is a board certified cosmetic surgeon for full body cosmetic surgery. In addition, he is a fellowship trained oculofacial reconstructive surgeon. This means that his specialty training after eye surgery, which was his first residency, was in advanced training in surgery of the eyelids, the orbit, the bones around the eye, the lacrimal tear duct system, and facial cosmetic surgery. Dr. Prasad says that this woman is directly guided very well by her doctor. Dr. Prasad has achieved a certain reputation for repairing complications of cosmetic eyelid surgery and one of the most common problems he sees is lower eyelid retraction which is due to improper cosmetic eyelid surgery and different thermal devices, specifically laser devices. The eyelid retracts downwards, or sometimes it everts out and retracts downward from surgery or from overuse of thermal devices. His job is to restore the integrity and the support systems that have been compromised from surgery or treatment. The evolution of the way to approach under eye lines has progressed over the past 20 years. In the beginning, doctors were using a full ablative, very aggressive CO2 laser which created very good results. Unfortunately, a lot of people also had complications. They were also using the erbium laser for many years. This same erbium laser is now been retooled in different ways for various fractionated devices, but has less thermal energy. Part of treating wrinkles does require a certain amount of heat, but it must be just a certain amount. In the past 2 years, Dr. Prasad has been using fractional CO2 laser. Dr. Prasad emphasizes that it’s very important to understand that fractional CO2 laser is not a solution in itself, but how the doctor uses it that makes a certain difference; experience, method and technique are critically important as it’s not a generic procedure. At the same time, he also tried to address pigmentation issues to get optimal results. Patients should define success by improvement, not by achieving porcelain-smooth skin. Trying to get porcelain-smooth skin under the eyes is very difficult and impractical as the skin is only a half a millimeter in thickness and it can only be heated so much. Dr. Prasad has developed a combination approach in his practice. He uses platelet-rich plasma (PRP) which is derived from the patient’s own blood. It has healing abd growth factors that are needed for wound healing. It works very well in synergy with thermal energy whether it’s fractional CO2 laser or with Pellevé. PRP can be an ongoing treatment to help improve the skin quality. Dr. Prasad explains that when dealing with a structure that is very thin, doctors really have to be strategic. Many people go to many different places to get treatments from different heating devices which ultimately leads their skin to get thinner and crêpey. They have to learn the balance of this risk of hypopigmentation. As a general rule of thumb, there’s a critical amount of depth needed to get a certain level of result, but the deeper one goes, the higher the risk. So it’s always a balance.
