Low grade porocarcinoma:hidradenocarcinoma Phillip McKee, Antonina Kalmykova & CSD Health Care

This interesting case comes from the foot of a middle aged female. We though that it represents a well differentiated porocarcinoma or hidradenocarcinoma since it is associated with inguinal lymph node metastasis. We were struck by the rather bland cytology of both the primary tumor and the nodal metastasis. It does bring into question the alternative diagnosis of so-called benign metastasizing hidradenoma. However, we are both very uncomfortable with such a diagnosis and decided that it would be better regarded as a low-grade (in terms of cytology) carcinoma. There is a suggestion in the primary tumor that it might well be arising from the epidermis and so porocarcinoma might be the best diagnosis although in the deeper reaches it certainly has a hidradenoma-like appearance.
