Case 196: Manual of CTO PCI - Parallel wiring

A patient was referred for PCI of an ostial LAD CTO that had an ambiguous proximal cap. IVUS helped clarify proximal cap ambiguity. An OTW Venture catheter enabled advancement of a Gaia Next 2 wire into the proximal LAD. De-escalation was done to a Fielder XT-A but the wire subsequently went outside the prior stent. Using the parallel wire technique we advanced a Gaia Next 3 within the prior stents into the distal true lumen. A nice final result was achieved after stenting from the left main into the LAD.
