Have you ever had a patient present with a peripheral corneal ulcer, and then ask yourself “Is that sterile or infectious?” And then you treat the ulcer aggressively but it just doesn’t resolve? How about a patient with a herpetic corneal lesion that is also not responding well to your treatment? Nonhealing corneal lesions can be a nightmare in the office. If the cornea doesn’t heal quickly, patient can continue to suffer from discomfort, their vision can decline that could be irreversible if the cornea scars, and worst of all, the infection could progress to something more serious like endophthalmitis. On top of that, it can really stressful to have a patient with a serious keratitis that is not improving and wonder how quickly you can get them to a corneal specialist to avoid irreversible damage. This becomes trickier when the patient is self-pay or unable to travel to the referred location; and if the vision worsens AFTER they see you, now you’re worried about a potential lawsuit.
