The main reason we are worried about neonatal hypoglycemia is the risk of brain injury-also called NHBI-neonatal hypoglycemic brain injury. This is highly likely where the hypoglycemia is severe or prolonged, especially in a setting where the baby is unable to utilise alternative fuels, or has burnt all options (like in an asphyxiated baby). The baby might present with seizures, lethargy and encephalopathy. MRI typically shows changes in the parieto-occipital cortex with lack of grey and white matter differentiation. More severe cases may show a generalised involvement as well. It is very important to be alert to risk factors, screen and manage early. And in the babies not at risk, it is still very important to support breastfeeding so we don't end up in unfortunate cases like the one I discussed. Please share.
