What Is Survivorship? A person with cancer from the time of diagnosis to the end of life, including family members, faces contentious definitions concerning agency. NCI defines survivorship as focusing on a person's health and well-being from diagnosis until the end of life. 10:49 Goals of Survivorship Survivorship goals include both clinical and unique individual patient goals like healthy living, goal setting, mindfulness, meaning, and issues concerning sexual health, hormones, and body image. Sometimes patient and clinical goals do not align. Past literature tends to focus only on clinical goals, missing interpersonal and unique patient goals. 15:45 Pediatric & Adolescent Survivorship in Thyroid Cancer Many online resources and survivorship plans are available yet underutilized. Stigma labeling thyroid cancer as "a good cancer" may inhibit further questioning. Barriers also include treatment in endocrinology departments instead of oncology. 20:31 What Is A Survivorship Plan? A survivorship plan addresses both supportive care and practical clinical care needs, including clinical outcomes, surveillance, and psycho-social needs. It is vital to start planning before the transition begins. 27:13 Quality of Life Quality of life is an individual's perception of their life in relation to their goals and values, differing from health-related quality of life, which includes physical, mental, and social well-being. Factors affecting quality of life include fear, daily medication, survivor's guilt, and challenges related to being labeled with "good cancer." 33:22 Transition From Childhood to Adulthood Up to 60% of pediatric cancer survivors transitioning to adult services experience disease-related complications. Questions about when adulthood begins, who should initiate the transition, and necessary policy and practical changes are prevalent. 37:33 Dr. Bauer’s Discussion: HrQOL & Transition of Care Despite better prognosis, quality of life (QOL) for pediatric cancer may not improve. Quality of life and transition plans lack standardized outcome measures.
