Pathology, Clinical manifestations, Diagnosis and Treatment Hereditary spherocytosis is an inherited defect in RBC cytoskeletal proteins (such as spectrin and ankyrin) or proteins that anchor the cytoskeleton to the RBC membrane (such as band 3, band 4.1, and band 4.2). A loss of portions of the RBC membrane results in circular RBCs (spherocytes) that have difficulty passing through the spleen, resulting in hemolysis. The classic triad of presentation in HS is mild anemia, intermittent jaundice, and splenomegaly. The peripheral blood smear shows varying numbers of spherocytes. The CBC shows a normocytic anemia (decreased hemoglobin, normal MCV), an increased RDW, and an increased MCHC. The presence of spherocytes can be confirmed by an osmotic fragility test.
